Every support journey looks a little different but we always start with getting to know you.

Your support coordinator on the Gold Coast will take time to understand you, your goals and what you want from your NDIS plan. We’ll make sure you know what’s possible and help you see how your plan can support your lifestyle.

Once we know what matters to you, we’ll link you with local services such as in-home help, transport or skill-building programs. We’ll handle the details, explain each step and check that everything feels right for you.

We’ll meet regularly to review your plan, update services and make changes if needed. With our Gold Coast support coordination, you’ll always know what’s happening and who to call if you need help.

Support coordination helps you understand your NDIS plan and use it confidently. It’s about making sure every part of your plan works for you. Your coordinator will explain how your funding works, help you choose the right supports and link you to the right providers.

You’ll have someone who checks in, answers questions and helps you build independence step by step. With support coordination on the Gold Coast, you can feel more organised and in control of your daily life.

You can use this service if your NDIS plan includes support coordination. If you’re not sure, we’ll help you check your plan and explain what each section means.

If support coordination isn’t included, we can guide you through how to request it at your next plan meeting.

There are three levels:

  1. Support connection helps you start using your plan and link with local services.
  2. Support coordination gives you regular help managing providers and helps to build the skills to coordinate independently.
  3. Specialist support coordination is for people with higher needs who require more guidance.

Your NDIS planner will decide which level is best for you . Your support coordinator will help line up the right supports for your needs.

Support coordination is only available if it’s part of your NDIS plan. If it isn’t, there are still other ways to get support. We can explain community programs, local supports and other services you may be able to use. Our team can also help you get ready for a plan review so you can add NDIS support coordination in the future.

We take the time to get to know you and what matters most in your life. We’ll talk about your interests, challenges and goals, and then help you choose supports that match. That might include in-home help, therapy, employment options or community activities

We’ll connect you with local providers on the Gold Coast and make sure your services fit your lifestyle. You can always talk to us if something isn’t working. We’ll help you make changes that feel right.
