How it works

Our Adelaide support coordination services are here to make your NDIS journey clear, flexible and full of choice. Here’s how it all comes together:

1

We start by sitting down together to learn what’s important to you. You can tell us about your everyday life, your goals and what you’d like more help with. We’ll then go through your NDIS plan in plain language so it’s easy to understand. This first step helps us build a clear picture of your needs and how we can best support you.

2

Once we know your goals, we’ll work with you to connect with supports that make sense for your life. This might include in-home help, personal care, employment or learning opportunities, or joining community programs around Adelaide. We’ll make sure every service matches your needs, interests and comfort level so you feel supported every step of the way.

3

Things change and your plan should change with it. We’ll check in with you to see how things are going and make updates when needed. If your goals shift or you want to try something new, we’ll adjust your supports to match. We’ll also help keep track of your plan and budget so you can confidently stay on top of everything.

Meet our team

Jackie Hall (SA)

I became a support coordinator to help people understand the NDIS and explore what’s possible. I love being part of my clients’ stories and helping create opportunities and new connections. Together, we identify goals and plan practical steps to achieve them. Outside of work, I enjoy travelling, exploring new places and learning about different cultures.

FAQs

Support coordination helps you understand and use your NDIS plan confidently. It makes your supports work together and fit your lifestyle. Your NDIS support coordinators in Adelaide will help you find service providers, manage your plan funding and guide you through any changes. We’ll also help you build skills so you can take charge of your plan in the future and make choices that feel right for you.

You can get support coordination services if they’re included in your NDIS plan. If you’re not sure, our team can help you check your plan and explain what’s covered. We can also help you ask for support coordination in your next plan review if it’s not there. Even if you’re new to the NDIS, we’ll guide you step by step so you understand what you can get and how to get started.

There are three main levels. Support connection helps you build skills to understand and manage your plan. Coordination of supports goes further and helps you organise and connect with services that fit your daily life and goals. Specialist support coordination is for people with higher needs and involves more planning. Our team can explain which level fits your lifestyle and help you get the most out of your funding.

Support coordination is funded through the NDIS so you’ll need it listed in your plan. If it’s not included, there may be other ways to get help. You can self-fund our services or we can connect you to other disability supports that match your needs. We’ll help you explore all your options so nothing feels too hard or confusing.

We take time to listen to you and learn what matters most. Then we look for supports that help you live comfortably and reach your goals, whether that means help at home, job support, therapy or joining social activities. Everything we recommend will fit your plan, your goals and your comfort.

Living independently is different for everyone. For some people, it means having help with cooking or cleaning. Others might want help getting to work or joining local activities. We’ll help you shape what independence means to you. With support coordination, you can build the skills and confidence to make choices, meet new people and enjoy a comfortable, connected life.

An NDIS support coordinator in Adelaide is your guide to understanding and managing your plan. They help you set goals, link with services and solve problems when things change. They also help you learn how to manage your plan more independently.

It’s normal for support needs to change as life moves on. When that happens, we’ll talk to you about what’s different and review your plan together. Maybe you’ve reached one goal and want to set a new one or your routine has changed. We’ll update your supports so they continue to suit you. You’ll never have to go through those changes alone. We’ll be there to help adjust everything so it still works for your life.
