How it works

Getting started with support coordination in Wollongong is easy:

First, we’ll chat with you about your NDIS plan, goals and what kind of help feels right. You can tell us what you’d like to achieve at home, in your community or in your daily routine. This helps us understand your needs and what support will make things easier for you every day.

Your coordinator will help find providers across the Illawarra. This can include help at home, therapy, employment support and community programs. We’ll explain all your options and make sure everything is clear and in line with your NDIS plan.

As your needs or goals change, we’ll check in to make sure your plan still suits you. We’ll help make updates and keep things running smoothly, so you always have the right support behind you.

Our support coordination services can take place in your home. We can also provide support coordination services online through telehealth appointments.

Support coordination helps you use your NDIS plan to achieve your goals. Your coordinator will connect you with local service providers, explain how your plan works and help you feel confident managing it day to day.

You can access our service if your NDIS plan includes support coordination funding. If you’re not sure, we’ll help you check your plan and understand your options. Even if you don’t have it yet, we can show you how to ask for support coordination services in the future.

There are three levels:

    1. Support connection helps you start using your plan and link with local services.
    2. Support coordination gives you regular help managing providers and helps to build the skills to coordinate independently.
    3. Specialist support coordination is for people with higher needs who require more guidance.

Your NDIS planner will decide which level is best for you. Your support coordinator will help line up the right supports for your needs.

Yes. You can self-fund support coordination services or find other funding options. We’ll explain the choices and help you find the best path to get started.

We take time to understand your needs, goals and comfort. Then we’ll connect you with providers in the Illawarra who offer personal care, daily living help, employment, social supports and more.

Living independently means having the support to make your own choices. It could be learning new skills at home, enjoying community events or having someone there to help with your everyday routine.

Your support coordinator helps make the most of your NDIS plan. They explain how things work, connect you with local supports and help you stay on track toward your goals. They’re your go-to person for advice, updates and everyday guidance.

When your needs or goals change, we’ll help you review your plan and swap supports where needed. Your coordinator will be there for each step, making sure you have what you need to feel supported and confident.
