With us, connecting with the right NDIS support coordination providers in Sydney is easy. Here’s how we’ll work together to set up supports that truly suit you:

First, we’ll chat about your goals and what you want from your NDIS plan. You can share what’s going well, what’s been tricky and where you’d like more support. This helps us understand what matters most to you, whether that’s finding community activities, improving daily routines or building independence.

Your support coordinator will help you explore service options across Sydney. Together, we’ll make a flexible plan that connects you with the right NDIS providers and community supports. You’ll always have a say in what happens next and we’ll make sure you understand how each choice fits into your plan.

Once your supports are in place, we’ll check in every now and then to see how things are going and make changes when needed. If your goals shift or you want to try something new, your coordinator will help update your plan so it continues to cover your needs

Support coordination helps you get the most out of your NDIS plan. Your support coordinator will guide you and connect you with local service providers based on your needs and goals. You’ll have someone who understands how the NDIS works and can explain things clearly so you can stay in control of your support.

You may be eligible if your NDIS plan includes funding for support coordination. If you’re not sure, we can help you check your plan and find out what level of support coordination you can get. Even if it isn’t listed in your plan, we can help you find other options and guide you through what to do next.

There are three levels:

  1. Support connection helps you start using your plan and link with local services.
  2. Support coordination gives you regular help managing providers and helps to build the skills to coordinate independently.
  3. Specialist support coordination is for people with higher needs who require more guidance. 

Your NDIS planner will decide which level is best for you . Your support coordinator will help line up the right supports for your needs.

Yes, it’s possible. You can self-fund your support coordination or use other funding options. Many people start with self-funding while waiting for NDIS plan reviews or extra funding. Our team can explain the choices and help you decide what’s best for your situation.

We listen to what’s most important to you including your comfort, your level of independence and your goals. Your coordinator will recommend providers who match your interests and needs. We’ll also help you understand what each provider offers so you can make confident choices about who supports you and how.

Living independently can mean many things like learning new skills, managing your own schedule, cooking your own meals or joining local activities that make you happy. With our help, you’ll have support that makes each day easier and more enjoyable while still giving you the freedom to make choices about your own life.

A support coordinator acts as your guide through the NDIS. They explain your plan, help you find the right supports and connect you with providers who can help you reach your goals. They also help you build the confidence to manage your plan more independently. If problems come up, your coordinator can help you solve them quickly so you stay on track.

Things change and your NDIS plan can change too. If your needs, goals or living situation shift, we’ll help you review your plan and update your supports. Your coordinator will take care of the details, make sure your services still suit you and guide you through updates or reviews with the NDIS.
