How it works

Getting started with NDIS support coordination in Western Sydney is easy. We’ll help make the NDIS easier to understand and make sure you’re getting the right supports for your needs.

First, we’ll have a friendly chat about your goals, what’s important to you and what kind of services you’d like from your NDIS plan. This will help us understand what good support looks like to you, whether it’s learning new skills, joining local activities or being more independent at home.

Next, your coordinator will help you find services and supports across Western Sydney. This might include in-home support, therapy, employment help or community programs. You’ll always have a say and we’ll make sure you understand how each service fits into your NDIS plan.

Once your supports are set up, we’ll keep in touch. Life changes and your plan should change with it. We’ll have a look at how things are going and update supports whenever you need them. That way, your plan always works for you.

Our support coordination services can take place in your home. We can also provide support coordination services online through telehealth appointments.

Support coordination helps you use your NDIS plan in a way that makes sense for you. Your coordinator will guide you through each part of the plan, connect you to services and help you manage your supports so you can focus on living your best life.

You can access support coordination if it’s included in your NDIS plan. If you’re not sure, we can help you check. Our team can explain what’s covered, what level of coordination you might have and what to do if you want to include it in your plan.

There are three levels:

  1. Support connection helps you start using your plan and link with local services.
  2. Support coordination gives you regular help managing providers and helps to build the skills to coordinate independently.
  3. Specialist support coordination is for people with higher needs who require more guidance.

Your NDIS planner will decide which level is best for you. Your support coordinator will help line up the right supports for your needs.

You can still talk to us if support coordination isn’t in your current plan. Some people self-fund their support coordination services or get other kinds of funding. We’ll explain what’s possible and help you work out the next steps.

We take the time to understand your goals and what feels right for you. Then we’ll connect you with local providers in Western Sydney. We’ll make sure you’re connected with someone who helps you feel comfortable, supported and in control of your choices.

It can mean many things like learning new skills at home, joining community activities or managing your own schedule with help from our team.

A support coordinator helps you make the most of your NDIS plan. They explain your options, help manage your plan, link you with the right providers and support you to stay on track toward your goals. You’ll always have someone in your corner who knows the NDIS and can help when things change.

If your goals shift, we’ll help you update your plan and services. Your coordinator will check in with you to make sure your supports still fit your needs. We’ll handle the tricky details and keep things running smoothly so you can focus on enjoying your daily life.
