What is support coordination?

Support coordination is when we support you to make the most of your NDIS plan. Our friendly support coordinators are here to support you to live your life’s adventure.

Together we will work to understand your NDIS plan, find the right services and make sure the supports you choose fit with your goals and lifestyle. You tell us what you need! We will always listen and make sure you have the support you want.

We will get to know you, your goals and your NDIS plan. Together we will understand your plan and work out what supports are right for you.

We will find the right services and providers with you, from in-home support to home and living and community activities. We will always listen and make sure you have the support you want.

We check in with you, review your plan and budget and support you through any changes so you can stay on track and live your life’s adventure with confidence.

I love building meaningful connections and seeing the difference it makes in people’s lives. Outside of work, I enjoy time with my dog, family and friends, exploring nature and relaxing at the beach.

FAQs about support coordination

Support coordination helps you understand and make the most of your NDIS plan so you can reach your goals. Your support coordinator will get to know you. They will work with you to find the right services, connect you with providers, and make sure your supports fit your needs and lifestyle.

You can access this service if support coordination is included in your NDIS plan. If you’re unsure, your coordinator can help you check your plan and guide you through the next steps.

There are three levels: support connection, coordination of supports and specialist support coordination. Which level is right for you depends on your individual needs and goals. We can support you to understand your options and match you to the right level of support.

No. NDIS support coordination is only for people who have an NDIS plan. We have lots of other services you can access without NDIS funding though!

We take the time to understand your wants, needs and goals. Then we connect you to services that best suit your lifestyle, whether that’s in-home support, assistance with daily living, employment, therapy or community activities.

Living independently means something different for everyone. For some people that means a lift to the shops, a social event or a night out with friends in one of our cars. For others it is having one of our friendly in-home support workers close by, day or night, to share the serious stuff and a few laughs together along the way.

A NDIS support coordinator’s job is to help you to understand and make the most of your NDIS plan so that you can reach your goals and live as independently as possible. They are experts in NDIS support coordination. They start by getting to know you. Then they find the right services and providers and make sure your supports fit your needs and lifestyle.

We know things change from time to time, and that is okay. When this happens, let’s chat about it. Then together will adjust your plan and services. Your support coordinator will be right by your side through every change to make sure your supports continue to work for you. Because life is better with a little back up.
